Application Forms:

Open Enrollment for the 2025-2026 school year begins on Dec. 4, 2024.

*Please contact the office to inquire about availability before submitting an application. You will find an Enrollment Application form for the 2024-2025 school year linked below, starting 12/4/24.

Please complete the application form and submit it to the Preschool Office. The Application Fee can be paid by check or Zelle, info@lacny.org

Once your child has been accepted into the program, we will send all forms to you electronically. The following forms which are required by NYS OCFS are also linked below, for your convenience, as is our Parent Handbook.
